Notice of Privacy Practices

This notice describes how protected health information about your child may be used and disclosed by Clementine ABA, and how you may access that information. Please review it carefully and retain a copy for your records.

Your Rights

  • Request access to or a copy of your child’s health records in paper or electronic form, subject to applicable verification procedures and any reasonable processing fees permitted by law.
  • Ask Clementine ABA to amend information you believe is inaccurate or incomplete; Clementine ABA reserves the right to deny such requests as permitted by law, subject to your right to submit a statement of disagreement.
  • Request confidential communications or specific restrictions on use or disclosure; Clementine ABA may decline such requests except in the narrow circumstances required by applicable law.
  • Receive an accounting of disclosures made within the prior six (6) years, where required by law, and a current copy of this notice upon request.

Our Uses and Disclosures

Clementine ABA may use or share protected health information without separate authorization to deliver treatment, operate the practice, bill for services, comply with legal and regulatory obligations, respond to public-health and safety requests, support quality-improvement and clinical-research activities, participate in audits, defend against legal claims, conduct fundraising, and engage business associates under written contract. Uses or disclosures not described in this notice will be made only with your written authorization, which you may revoke in writing at any time except to the extent action has already been taken in reliance on it.

Our Responsibilities

Clementine ABA is required by law to maintain the privacy and security of protected health information, to notify affected individuals if a breach involving unsecured information occurs, and to abide by the terms of the notice currently in effect. Clementine ABA reserves the right to amend this notice at any time and to apply the revised terms to all protected health information it maintains; the current version will be posted on this site, and your continued engagement with Clementine ABA services after posting constitutes acknowledgement of the updated terms.
